Donation Boxes
Form: Donation Box Application
Any placement of a donation box within the city limits of Houston must have a valid donation box permit.
Requirements: Applicants must:
- Submit Donation Box Application
- Submit site plan or map (for each donation box)
- Submit service plan (for each donation box)
- Pay appropriate fees (see schedule)
- Submit and maintain commercial general liability insurance with coverage of not less than $1,000,000. The city shall be listed as an additional insured.
Permit expiration: One year from date of issuance

2024-2025 donation box decal placement and image:
The administrator shall issue a decal to the applicant to display conspicuously on the donation box. This decal shall remain affixed to the donation box and visible on the outside of the donation box for the entire duration of the permit.
