Certificate Request Guidelines

• Make the request at least two weeks in advance of the date of your event
• Please include information that is 250 words in length in the form below and is relevant to the event / honoree.
• Any organization or individual can request a max of 10 certificates annually.
• If you would like Council Member Plummer to present the certificate at your event:
  • Make the request at least 30 days in advance of your event
  • Also complete the Event Request Form
