U.S. Passport Services

The Municipal Courts Department on behalf of the U.S. Department of State accepts passport applications for U.S. citizens who want to get a new passport. Citizens may apply in person using the DS-11 application only. We do not accept the DS-82 renewal application.
Am I Eligible to Schedule an Appointment?
You are eligible to schedule an appointment and apply in person with a DS-11 application if:
- You are applying for your first U.S. passport
- You are under the age of 16
- Your previous U.S. Passport:
- Was issued when you were under the age of 16
- Was lost, stolen, or damaged
- Was issued more than 15 years ago
- Was issued under another name and your name has now changed
If none of the above applies, you may be eligible to renew your passport by mail using the DS-82. The DS-82 cannot be accepted at our facility it must be mailed to the address on the form. We only accept the DS-11 application.
I Am Eligible!
These are the steps to apply in person (Electronic version of Citizenship or Id is not acceptable):
- Complete a passport application (DS-11), in black ink or typed prior to arrival. Passport Applications & Forms
- Bring the original citizenship document(s) and one photocopy of the document(s).
- Your original citizenship document(s) will be sent to the Passport Agency and will be returned back to you after you receive your passport. Eligible Citizenship Documents
- Bring the original identification and one single-sided copy of the identification. Eligible Identity Documents
- Bring your recent passport photo (Photo services are not provided at our facility)
- Photo must have been taken within 6 months of appointment Photo Requirements
- Bring two separate payments (Fees cannot be combined):
- City of Houston: Non-refundable $35 fee per applicant, payable by check, money order, or credit card. Make payment payable to CITY OF HOUSTON.
- U.S. Department of State: Make payable by check or money order. Make payment payable to U.S. DEPARTMENT OF STATE.
Minors
All legal guardians must be present or submit a consent form (DS-3053).
Original Valid ID is required for applicants 16 years and older.
