2024 - 2025 Win for Life
We want to make sure you have the tools and resources to achieve your health goals and live the healthiest life possible. The Win for Life 2024 - 2025 program makes it easy for you to save money by earning your medical plan rate discount for next year.
Employees and their covered spouses who complete and annual physical and online health assessment at MyCigna.com will earn the following:
- Employees – Medical rate discount of $600 annually
- Spouses – Medical rate discount of $300 annually
Healthy Awards funds can be used towards medical copays, dental copays, or a point-of-service for prescriptions.
Please Note: Healthy Award Funds earned during the current program year are not made available until the completion of the program year (May 1, 2024).
STEP 1: ANNUAL EXAM
Deadline: January 31, 2025
Employees and covered spouses, visit your provider for an annual physical between February 1, 2024 – January 31, 2025. Obtain biometric measurements for blood pressure, total and HDL cholesterol, height, weight, and waist circumference.
STEP 2: HEALTH ASSESSMENT
Deadline: March 14, 2024
Employees and covered spouses, use your biometric measurements to complete the online health assessment at MyCigna.com between May 1, 2-24 – March 14, 2025.

Actions: Complete at least two preventive care activities. All medical services must be with a Cigna healthcare professional and will be automatically reported through a claims submission, except where noted as self-reported.

Option B: Health Coaching
Available: March 1, 2024 – March 1, 2025
Achieve a health goal with the support of a telephonic, virtual or onsite health coach. Together, we will create a health plan to help you reach your personal health goal(s) on the following topics: diabetes, low back pain, osteoarthritis, heart disease, weight management, stress management, tobacco cessation*, physical activity, healthy eating and more.
Upon completion of the program, participants are eligible to earn a $50 gift card for the 2022 – 2023 Win for Life Program. (Annual physical and online health assessment must be completed before gift card is awarded).

Telephonic health coaching is also available with unlimited space by calling 1-800-997-1406. *If virtual health coaching is full or “sold out” you may still access telephonic coaching.
The $50 gift card is available for both virtual and telephonic health coaching. *Spouses are eligible for coaching but limited to telephonic coaching by calling 1-800-997-1406. Spouses are also eligible for the $50 gift card awarded after the completion of an annual physical and online health assessment.

Option C: Onsite/Virtual Health Education Programs
Available: May 1, 2024 – March 14, 2025
Actions: Attend or participate in a City of Houston facilitated onsite or virtual program.
Programs may include, but are not limited to:
- Multi-week lifestyle management programs
- Weight Watchers (WW)
- Kelsey Diabetes Care Program
- Department programs
- CityFit Onsite Exercise Program
- Fitbit challenges
- Others as defined by the City of Houston Wellness Team
Business days for auto-reporting: 30 days from completion
Average required completion time: 3-12 weeks

Option D: Achieve a Healthy Goal
Available: February 1, 2024 - January 31, 2025
Actions: Obtain two labs within the guidelines below. Note: All labs must be reported via a Physician fax form available on myCigna.com or done at a lab within Cigna’s lab network (LabCorp, Quest).
- Achieve a fasting blood sugar of less than 100 or non-fasting blood sugar of less than 140.
- Achieve a healthy cholesterol ratio of 5.0 (male) or 4.4 (female).
Business days for auto-reporting: 60-90 days from completion
Average required completion time: 3-4 months
If labs are done outside Cigna's lab network, a Wellness Screening Form must be submitted. The form can be found at myCigna.com under the "Related Links” menu on the Incentive Award page.
Option E: Tobacco Cessation Program
Available: March 1, 2024 – March 1, 2025
Actions: Attend an 8-week virtual group class.
- Group of 10 employees led by a Cigna Health Coach.
- Focuses on step-by-step plan for quitting tobacco and transitioning to a tobacco-free lifestyle.
- Free nicotine gum and patched provided.
- Program completion removes the tobacco user surcharge.
One-on-one tobacco coaching is only available telephonically.
